Welcome to Paragon Psychiatric Rehabilitation Program — a local, community-based program designed to help stabilize the population of psychiatrically diagnosed adults and allow them to reside and grow in the community.
We provide consistent, effective Rehabilitative Services that contribute to the stabilization and recovery from
debilitating mental health disorders by integrating closely with Social Workers, Discharge Planners, Housing Resources, Physicians, and family.

Social Workers and Discharge Planners:
Our experienced and professional staff understand the urgent need to discharge patients with supportive community resources in place. We are here for both YOU and the PATIENT. We partner with housing providers, counseling services, pharmacy support, and other critical connections, to facilitate a seamless transition from facility to community.
