Our Licensed Mental Health Professionals collaborate as a team with each client to create an Individualized Rehabilitation Plan designed to:

  • Help stabilize, maintain, and improve connection to community and family
  • Develop skills and identify strengths which foster improved health, wellbeing, functionality and social engagement
  • Improve an overall quality of life
  • Obtaining Employment and/or GED
  • Obtaining a basic identification document
  • Independent living skills
  • Understanding the public assistance programs
  • Addressing self-isolation
  • Understanding Mental Health and Self Advocacy
  • Improving communication and social skills

Services are delivered in a combination of community and clinic-based settings. Individual and group interactions serve to focus and build on the individual’s needs as well as the individual’s ability to integrate in the community.  

Individualized Rehabilitation Plans: (IRPs) created by Licensed Mental Health Professionals that focus on wellness, stability, and community integration while addressing:

  9. Reinforcement of Psychiatric skills. Seamless Progress Towards Personal Growth

Transportation: provided to support the client’s ability to remain in a community setting and receive program services

Community-based services: IRP content delivered by qualified counseling staff in community and clinic settings

A holistic approach: care which identifies and helps address co-existing diseases, behaviors, or conditions which have a negative impact on mental health

Collaboration: interaction with partners in care such as family, residential providers, physicians and other somatic health providers (where applicable and authorized), and other community-based entities
